Role:
The Nurse Practitioner (APRN) at Holton Family Medicine Clinic plays a critical role in delivering comprehensive primary care to patients of all ages. This position focuses on diagnosing, treating, and managing acute and chronic illnesses while promoting health and wellness within the community. The Nurse Practitioner will collaborate closely with physicians, nurses, and other healthcare professionals to develop individualized care plans that address patients' physical, emotional, and preventive health needs. By providing patient education and counseling, the role supports informed decision-making and encourages healthy lifestyle choices. Ultimately, this position aims to improve patient outcomes and enhance the overall quality of care within the Family Medicine Clinic setting.Minimum Qualifications:
Current and unrestricted Nurse Practitioner (APRN) license in the state of Kansas. Master's or Doctoral degree in Nursing from an accredited program. Certification as a Family Nurse Practitioner (FNP) by a nationally recognized certifying body. Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications. Minimum of 2 years clinical experience in a primary care or family medicine setting.Preferred Qualifications:
Familiarity with electronic health record (EHR) systems, preferably Athena Health. Additional certifications in geriatric care, pediatrics, or chronic disease management. Strong background in patient education and community health promotion. Participation in professional nursing organizations or continuing education activities.Responsibilities:
Conduct thorough patient assessments including history taking, physical examinations, and diagnostic testing. Diagnose and manage a wide range of acute and chronic medical conditions in patients across the lifespan. Develop and implement individualized treatment plans, including prescribing medications and therapies as appropriate. Provide patient education on disease prevention, health maintenance, and management of chronic conditions. Collaborate with interdisciplinary healthcare teams to coordinate patient care and ensure continuity. Maintain accurate and detailed patient records in compliance with legal and regulatory standards. Participate in quality improvement initiatives and stay current with evidence-based practices. Respond to patient inquiries and provide follow-up care to monitor treatment effectiveness.Skills:
The Nurse Practitioner will utilize advanced clinical assessment and diagnostic skills daily to evaluate patient health status and determine appropriate interventions. Effective communication skills are essential for educating patients and collaborating with multidisciplinary teams to ensure comprehensive care delivery. Proficiency in electronic health record systems enables accurate documentation and efficient management of patient information. Critical thinking and problem-solving abilities support the development of individualized treatment plans tailored to diverse patient needs.
