Job Description

Duties and Responsibilities:
Provide full-scope primary care services, including preventive care, acute care, and management of chronic conditions for patients of all ages. Deliver quality, evidence-based, patient-centered care in alignment with the health center's mission. Provide care for underserved populations, including patients with complex medical, behavioral, and social needs. Collaborate with care teams to coordinate referrals and follow-up care. Perform minor in-office procedures within the scope of training. Work collaboratively with other physicians, advanced practice providers, nurses, medical assistants, behavioral health professionals, and care coordination staff. Support integrated care models, including behavioral health and population health initiatives. Participate in community outreach and health education efforts as appropriate. Complete medical charts and documentation in compliance with the health center's policies and procedures. Attend internal and external meetings and participate on committees as deemed necessary by the Chief Health Officer and/or CEO. Regular attendance, punctuality and timely documentation are essential requirements of this job. Ability to travel between clinic sites. Other duties or responsibilities as assigned. Qualifications Must have active, unrestricted KS license as a Nurse Practitioner. Maintain an active and unrestricted KS DEA number. Current CPR (BLS) certification. Must be eligible for liability coverage via FTCA and private malpractice insurance carrier. Approval of Kansas Charitable Care Provider Application. Staff membership and privileges granted by the HPC Board of Directors (approval of credentialing and privileging applications). Ability to relate well with patients and other health care personnel. Ability to maintain appropriate clinical privileges. Ability to work in a fast-paced environment. Commitment to expanding access to care for the underserved. Knowledge of public health principles and practices, preferred. Ability to work with the Electronic Health Record system. Must meet vaccine requirements per FQHC regulations.
Education and Experience:
Master of Science degree in Nursing (MSN) or Doctorate of Nurse Practitioner (DNP) and certified Advanced Practice Registered Nurse (APRN). Minimum three (3) years' experience working in a community health center or similar environment, preferred
Typical Physical Demands:
Prolonged sitting or standing may be required. Ability to lift 25 pounds; occasionally up to 60 pounds. Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with normal medical office will be performed on a repetitive basis. The ability to distinguish letters and symbols, as well as the ability to utilize telephones, computer terminals, and copiers is required. Ability to work in fast-paced and non-traditional conditions, and irregular hours may be required. Exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, radiation, and other conditions common to a clinical environment may routinely be encountered. Ability to handle a busy schedule. Some non-traditional hours may be required (i.e., Saturday rotation). EOE Under the direction of the Chief Health Officer, the Nurse Practitioner will provide primary care services commensurate with training to individuals across the lifespan. The care will be appropriate for an outpatient, primary care community health center with a multicultural, multilingual, and economically diverse patient population. This role emphasizes high-quality, culturally competent, and accessible care to underserved populations while supporting the health center's mission to improve community health outcomes.
