Full Job Description Compensation:
Productivity / RVU-Based Position Summary KC Medical Care is seeking an experienced, autonomous Nurse Practitioner to provide high-quality primary care to patients in Assisted Living and Independent Living communities. This role requires strong clinical judgment, proactive patient management, and collaborative communication with facility staff to ensure continuous, comprehensive care for your assigned patient panel.
Key Responsibilities Clinical Care & Assessment:
Perform complete histories, physical exams, diagnostic interpretations, and treatment plans independently for assigned patients.
Panel Management & Facility Communication:
Serve as the primary clinical point of contact for assigned patients. Promptly respond to phone calls, emails, and urgent inquiries from facility nursing staff regarding clinical updates, medication management, and patient status changes.
On-Call & Follow-Up Coverage:
Manage routine and urgent triage calls during designated working hours/days for your patient panel to maintain care continuity and minimize unnecessary emergency transfers or physician escalations.
Diagnostic & Therapeutic Management:
Order and interpret diagnostic tests, prescribe pharmacologic and non-pharmacologic therapies, and adjust treatment modalities as needed.
Care Coordination:
Collaborate directly with care coordinators and facility staff to ensure timely delivery of ancillary services, therapy, and home health.
Physician Collaboration:
Work consultatively with collaborating physicians to manage complex, acute, or chronic medical conditions.
Documentation & EHR:
Maintain complete, timely, and accurate medical records, charts, and billing documentation in accordance with practice standards and coding guidelines.
Patient & Family Education:
Educate patients, families, and facility care teams on diagnoses, treatment goals, and medication regimens.
Required Skills & Qualifications Clinical Expertise:
Proven experience in geriatric care, adult primary care, or long-term care environments.
Communication & Triage:
Excellent diagnostic evaluation skills combined with strong, proactive communication habits with facility nurses and administration.
Autonomy:
High level of comfort exercising independent medical decision-making within standard care protocols.
Coding & EHR Competency:
Working knowledge of E/M coding, billing practices, and clinical documentation efficiency.
Licensure:
Active, unencumbered Nurse Practitioner license and DEA registration in the state of practice.
