In this role, you will provide comprehensive patient care across a variety of clinical settings, ensuring high-quality, compassionate service. You will collaborate closely with physicians, nurses, and other healthcare professionals to deliver personalized treatment plans, perform assessments, and manage acute and chronic conditions. • Conduct initial patient evaluations.
• Complete follow-up visits to assess progress.
• Conduct final patient visits.
• Maintain accurate, timely documentation.
• Communicate findings and recommendations with the clinical team Requirements
• Current licensure and certification appropriate to practice as a Nurse Practitioner in Kentucky.
• Strong assessment, communication, and documentation skills.
• A dependable, professional, patient-centered approach.

Proficiency in EMR and telehealth experience preferred. Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques. Knowledge of CPT coding, ICD-10/ICD-9 coding systems for accurate documentation and billing purposes. Excellent client communication skills with the ability to educate patients on health coaching and treatment options. Ability to work independently in a per diem capacity while maintaining high standards of patient safety and confidentiality.
