Job Description

Position Summary We are seeking a compassionate and experienced Psychiatric Nurse Practitioner to provide comprehensive mental health services to nursing home residents. This role combines on-site clinical rounding at multiple nursing facilities with telehealth consultations. The successful candidate will deliver direct patient care, manage psychiatric medications, coordinate with interdisciplinary teams, and utilize technology to expand access to mental health services. Primary Responsibilities
  • Conduct regular psychiatric assessments and evaluations at assigned nursing home facilities
  • Develop, implement, and manage individualized treatment plans for residents with psychiatric and behavioral health conditions
  • Prescribe and manage psychotropic medications with appropriate monitoring and documentation
  • Perform medication reviews, adjust dosages, and manage side effects in collaboration with the care team
  • Conduct telehealth psychiatric consultations and follow-up visits
  • Document all clinical encounters in the electronic health record system with accuracy and timeliness
  • Communicate assessment findings and treatment recommendations to nursing staff, physicians, and families
  • Participate in interdisciplinary team meetings and care conferences
  • Monitor resident progress and adjust treatment plans as clinically indicated
  • Maintain compliance with state and federal regulations, facility policies, and clinical standards
  • Respond to psychiatric emergencies and behavioral crises in nursing home settings
  • Provide education and consultation to nursing home staff regarding resident psychiatric care Required Qualifications
  • Master's Degree in Nursing (MSN) or equivalent with specialization in Psychiatric/Mental Health Nursing
  • Current licensure as a Registered Nurse (RN) in Louisiana
  • National Certification as a Psychiatric Nurse Practitioner (PMHNP-BC or equivalent)
  • Prescriptive authority in Louisiana
  • Minimum 2 years of psychiatric nursing experience
  • Current CPR certification (ACLS preferred)
  • Proficiency with electronic health records and telehealth platforms
  • Strong clinical assessment, diagnostic, and therapeutic skills Preferred Qualifications
  • Experience in geriatric psychiatry or long-term care settings
  • Experience providing telehealth services
  • Knowledge of behavioral health in medically complex populations Essential Skills & Competencies

  • Travel between multiple nursing home facilities
  • Ability to lift and move up to 25 lbs regularly
  • Ability to stand and walk for extended periods
  • Visual acuity sufficient to read medical charts and electronic records
  • Ability to communicate verbally and in writing

Pay:
$115,000.00 - $130,000.00 per year
