Nurse Practitioner - Primary Care | Join a Growing Community-Focused Clinic

Heinen Wellness of Jennings is seeking a Nurse Practitioner to help develop and grow a patient-centered primary care clinic serving Jennings and the surrounding communities. Our focus is on continuity of care, chronic disease management, and building meaningful, long-term relationships with patients and families. This is not a high-volume, quick-turn environment. We are building a stable, relationship-driven practice rooted in the community, and we are looking for a provider who values long-term impact and continuity of care. What We're Looking For Licensed Nurse Practitioner in the state of Louisiana Certification by AANP, ANCC, or equivalent Strong interest in outpatient primary care Commitment to building long-term relationships with patients Ability to work independently while collaborating with a physician-led team This role is ideal for a provider who values stability, continuity of care, and becoming part of a close-knit community. Preference will be given to candidates with ties to the Jennings area or those interested in establishing long-term roots in the community.

Key Responsibilities:

Perform comprehensive history and physical examinations Diagnose and manage acute and chronic conditions Develop and implement treatment plans Order and interpret diagnostic tests Provide patient education and preventive care Coordinate care and make appropriate referrals Maintain accurate and timely documentation Compensation & Growth Opportunity Up to $120,000 per year based on experience Up to 3% bonus based on clinic performance Opportunity for increased compensation as patient panel grows Ability to play a key role in shaping a growing clinic Why Heinen Wellness?
