Job Description

Fast Pace Health seeks a Primary Care Provider to deliver comprehensive, patient-centered care in our rural community clinics. As a Nurse Practitioner or Physician Assistant, you will perform assessments, diagnose and manage acute and chronic conditions, and provide preventive and wellness services for diverse patient populations. You'll collaborate closely with a supportive, multidisciplinary team in a fast-paced, innovative environment that values compassion, excellence, and community impact. Responsibilities Provide comprehensive primary care, including assessment, diagnosis, and treatment of acute and chronic conditions for patients across the lifespan. Order, interpret, and act on diagnostic tests, including labs and imaging, following evidence-based guidelines. Deliver preventive care, wellness visits, and patient education to support long-term health and disease prevention. Document all encounters accurately and efficiently in the Electronic Medical Record (EMR) system. Collaborate with physicians, nurses, and other team members to coordinate care and ensure high-quality patient outcomes. Manage follow-up care, referrals, and care transitions within the Fast Pace Health network and external partners. Perform common in-clinic procedures such as wound care, suturing, and basic orthopedic management as appropriate to training. Ensure compliance with clinical protocols, regulatory standards, and organizational policies. Participate in quality improvement, training, and professional development activities. Contribute to a culture of compassion, respect, and community-focused care in a fast-paced clinical setting.
