Your team includes:
Gather Guides — community-based partners who address social determinants of health and connect patients to resources Medical Technician Partners (EMTs/Paramedics) — deployed into patients' homes for post-discharge follow-ups, chronic condition monitoring, and home safety assessments, reporting directly back to you Medical Assistants, Patient Benefits Partners, and Practice Operations Partners — handling the administrative and clinical support so you can focus on care Schedule flexibility built in Administrative days can be completed remotely. A typical week Clinic and telehealth visits with your panel (no double-booking, no 10-minute slots) Care team huddles with Guides and Medical Technician Partners Remote administrative time for documentation, care coordination, and team communication Home-based care coordination supported by your MTP team (you direct; they deploy) What we're looking for
Required:
Licensed Nurse Practitioner or Physician Assistant with primary care experience Active Massachusetts medical license, or ability to obtain one prior to start Clinical proficiency in evidence-based primary care Ability to connect authentically with patients from diverse backgrounds Comfort with EHR and care technology platforms Strong ethics and discretion in complex, sensitive situations Collaborative spirit — we work without hierarchy and welcome all perspectives
