The Nurse Practitioner will work collaboratively with physicians and other healthcare professionals to ensure comprehensive patient care. Duties Conduct thorough patient assessments, including medical histories and physical examinations. Diagnose and manage acute and chronic health conditions. Develop and implement individualized treatment plans based on patient needs. Provide health coaching to patients to promote healthy lifestyle choices. Collaborate with interdisciplinary teams in an outpatient rehabilitative setting. Maintain accurate medical records.
