Operating within a patient-centered medical home model, CHC offers integrated multidisciplinary care across multiple locations, ensuring accessibility and cultural competence for diverse populations. Job Summary We are seeking an experienced Nurse Practitioner to provide comprehensive primary care with an emphasis supporting gynecological services, while promoting overall wellness to patients across their lifespan. Your expertise will help empower patients through education, preventive care, and personalized treatment plans in a welcoming environment committed to excellence. Duties Conduct comprehensive gynecological examinations, including pelvic exams, Pap smears, and breast assessments.. Collaborate with Gynecologists and multidisciplinary teams to coordinate care for patients with chronic conditions like osteoporosis or menopausal symptoms. Educate patients on reproductive health, disease prevention, and lifestyle modifications tailored to individual needs. Document all patient encounters and coordination of care in EMR Requirements Certified Nurse Practitioner, with current MA Licensure Demonstrated experience supporting gynecological services Proficiency with electronic health records systems, ideally EPIC Experience working with diverse populations Ability to perform patient exams and assessments effectively while demonstrating compassionate communication skills.
Pay:
$120,000.00 - $140,000.00 per year
