Nurse Practitioner or Physician Assistant - Paneled PCP - Milford, MA Optum 401(k), relocation assistance United States, Massachusetts, Milford Aug 03, 2026 Milford, MA | Reliant Medical Group Reliant Medical Group, part of the Optum family, is seeking an experienced Nurse Practitioner or Physician Assistant to join its Internal Medicine team in Milford. This role offers the opportunity to independently manage a primary care panel while practicing in a collaborative, value-based care environment supported by advanced technology and a multidisciplinary care team. Position Highlights Independently manage a panel of primary care patients
Provide comprehensive preventive, acute, and chronic disease management
Collaborate with RNs, pharmacists, behavioral health, specialists, and population health teams
Support shared decision-making and whole-person care
Participate in a team-based primary care call structure
Utilize DAX AI ambient scribe technology and EPIC tools to reduce documentation burden

Required Qualifications
Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA)
Must have at least 3 years of paneled APC experience
Unlimited Licensure in the state of Massachusetts
Active DEA License Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
