Part-time - Nurse Practitioner or Physician Assistant Optum 401(k) United States, Massachusetts, Sterling Sep 09, 2026 Advanced Practice Clinician (NP/PA) - Skilled Nursing Facility | Leominster, MA Join Reliant Medical Group, part of Optum, as a Nurse Practitioner or Physician Assistant providing value-based care to residents in a skilled nursing facility setting. This role focuses on caring for older adults across skilled rehabilitation, long-term care, and palliative care populations while working closely with physicians and interdisciplinary care teams.
Highlights:
Skilled Nursing Facility position in the Leominster, MA area
0.6 FTE (24 hours/week), Monday-Friday schedule
Care for skilled rehab, long-term care, and palliative care patients
Collaborate with physician partners and multidisciplinary teams
Manage complex geriatric conditions, chronic illnesses, and post-acute care needs
Participate in goals-of-care discussions with patients and families
Geriatric, skilled nursing, or palliative care experience preferred
Minimum 2 years of APC experience preferred

NP or PA with Massachusetts licensure (or ability to obtain)
National Board Certification
BLS certification required The salary range for this role is $109,500 to $164,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
