This role focuses on caring for older adults across skilled rehabilitation, long-term care, and palliative care populations while working closely with physicians and interdisciplinary care teams.

Highlights:

Skilled Nursing Facility position in the Leominster, MA area

0.6 FTE (24 hours/week), Monday-Friday schedule

Care for skilled rehab, long-term care, and palliative care patients

Collaborate with physician partners and multidisciplinary teams

Manage complex geriatric conditions, chronic illnesses, and post-acute care needs

Participate in goals-of-care discussions with patients and families

Geriatric, skilled nursing, or palliative care experience preferred

Minimum 2 years of APC experience preferred

BLS certification required The salary range for this role is $109,500 to $164,000 annually based on full-time employment.
