Nurse Practitioner (NP) - Flexible Schedule | NP-Owned Practice | Sign-On Bonus If you are a compassionate and dedicated Nurse Practitioner looking for a fulfilling career with autonomy, flexibility, and strong patient relationships , we would love to have you join our team! At MyHome Healthcare , a Nurse Practitioner-owned practice, we provide mobile, home-based primary care to older adults in Massachusetts and Southern New Hampshire. We care for patients in Assisted/Independent living communities and private homes—focusing on quality, not quotas.

Responsibilities:
Conduct monthly wellness visits to your own patient panel in the comfort of their home Maintain positive relationships with community directors/charge nurses Foster open communication with patients and/or their family members Work collaboratively with the MyHome Healthcare team Order and interpret diagnostic tests Diagnose and treat various chronic/acute conditions and illnesses Maintain accurate records and schedules Educate patients and their family members on conditions, disease, prevention and treatments Oversee patients' overall health Cover calls regarding your patients during normal business hours (M-F, 8:00am-5:00pm) and weekend calls on a rotating coverage schedule Weekend call coverage on a rotational basis
Qualifications:
Adult Gerontology Nurse Practitioner preferred, will consider other certified NPs with experience in the geriatric population Home health experience preferred Active DEA and MCSR License in the state of MA Reliable vehicle and clean driving record
