Job Description
Full-Time Family Medicine Nurse Practitioner Opportunity LifeBridge Health is seeking a full-time Family Nurse Practitioner to join our Grace Medical Center Ambulatory Services team supporting population health initiatives in Baltimore City and Baltimore County . The qualified Nurse Practitioner will collaborate on the development of growing population health services and community partnerships. The Opportunity This full-time position will provide primary care services both in the community through the LBH Mobile Van and within Grace Medical Center's Family Medicine practice. The selected Nurse Practitioner will visit schools and other community partners throughout Baltimore City and Baltimore County to deliver preventative and primary care services, including well-child and adult wellness visits, sports physicals, school vaccinations, chronic disease management, and primary and sick care. The NP will also see patients in the Family Medicine practice to support practice growth, expand access, and established continuity of care. Remote access to EMR system, laptop computer, and printer when on the Mobile Van. No on-call responsibilities and no regularly scheduled evening or weekend hours; however, occasional weekend availability may be required to support community events, outreach activities, or special vaccination initiatives. Identify patients needing ongoing primary care services and connect them with Grace Medical Center's Ambulatory Services and Family Medicine practice to support continued care and continuity within the health system. license (or obtainable) and current CPR license.
