Physician Assistant or Nurse Practitioner Physical Medicine and Rehabilitation

Job Description

We are seeking an experienced advanced practice provider (APP) to lead our physiatry consultation service in a skilled rehabilitation setting. Responsibility includes rounding on patients, managing medical barriers, and collaborating with therapy teams to monitor functional progress. Participate in weekly interdisciplinary meetings to address discharge barriers and patient care strategies. Ideal candidates have leadership experience, a background in geriatrics, and 2+ years in rehabilitative or acute care settings. Requirements include active Physician Assistant or Nurse Practitioner license and experience with geriatric populations.
