Clinical Nurse Practitioner or Physician Assistant Provides primary patient care at Bear River Health, in an SUD residential treatment facility.

ENTRY REQUIREMENTS

Clinical Nurse Practitioner:

• Master's prepared in Nursing.
• Licensed as a Nurse Practitioner in the State of Michigan.

Graduate of MSN or

DNP CCNE

accredited FNP or Adult Health Nurse Practitioner program.

• Five years of clinical experience in general nursing. Certification through ANCC or AANP within one month of employment.
• Meets all Core Competencies of Practice for Nurse Practitioners and Population-Focused Competencies set forth by the AACN.

Or Physician Assistant:

• A Master of Science degree in Physician Assistant Studies from an ARC-PA (Accreditation Review Commission on Education for Physician Assistants) accredited institution.
• Certification as a Physician Assistant by the AAPA (American Academy of Physician Assistants)
• Board certification as a Physician Assistant
• Licensure as a Physician Assistant by the State of Michigan
• Candidate must apply for and be granted appropriate clinical privileges through normal credentialing processes before rendering services
• Previous experience is desirable.
• Maintains certification as a PA.
• Is a self-directed professional with the ability to make independent judgments in providing direct patient care, educating and supervising clinical staff, and coordinating programs.
• Good judgment and problem-solving skills.
• Excellent written and verbal communications skills.
• Ability to make quality, independent medical decisions.
• Time management and organizational skills.
• BCLS certified.

ORGANIZATION

Reports to Medical Program Director and Supervising Physician.

• Provides direction to RN's, LPNs, Medical Assistants, and/or other categories as appropriate.
• Works collaboratively with physicians and all members of the patient care team.

Standards of care: 1. Manage chronic conditions and acute episodic illnesses and minor injuries, using best practices, i.e., evidence-based practice 2. Obtain relevant health and medical histories from patients and/or caregivers 3. Perform physical exams appropriate for age and history 4. Identify medical and health risks and needs 5. Order, conduct and interpret appropriate diagnostic tests 6. Formulate the appropriate plan of care based on history, physical exam, clinical findings and any diagnostic findings 7. Develop care plans and communicate plan to patient, their care giver, or family 8. Perform procedures within the scope of expertise (e.g., I&Ds, suturing wound closure, etc.) 9. Identify non-pharmacologic interventions and prescribe non-pharmacologic therapies; identify pharmacologic interventions and prescribe pharmacologic therapies 10. Provide relevant patient education 11. Make appropriate referrals to physicians and other health professionals and community agencies 12. Participate in quality improvement review on a periodic basis, including systematic review of records and treatment plans, helps disseminate findings and implement plans for improvement 13. Serve as an educational resource for ancillary staff 14. Complete and maintain written electronic records in a timely fashion and in compliance with Bear River Health standards

The primary responsibility of the nurse practitioner is to promote and maintain health in all age populations; and to prevent disease and illness exacerbation. 8. Is a part of the maintenance of the Primary Care at Bear River Health . This may involve education, home visits, group visits, case management, pay for performance, and quality indicators. 9. Assists in policy and procedure development for Bear River Health operations and clinical interventions. 10. Maintains visibility within the community as a primary health care provider. Serves as a role model to clinic staff, faculty, and the community at large. 11. Psychiatric and SUD experience preferred. 12. Performs other duties and responsibilities as assigned.
