Nurse Practitioner Advanced Care Visiting Physicians Farmington Hills, MI Job Details Part-time $60 - $100 an hour 1 day ago Benefits Flexible schedule Qualifications Driver's License Nurse Practitioner (NP) Advanced practice provider Prescribing medications Full Job Description Nurse Practitioner - Home-Based Primary Care / Visiting Provider Advanced Care Visiting Physicians (ACVP) is seeking an experienced and motivated Nurse Practitioner (NP) to join our growing home-based medical practice. This is an excellent opportunity for an NP looking for a highly flexible, part-time position that can supplement an existing job or practice. We are initially looking for coverage approximately 2-4 days per month, with the opportunity for additional days and a larger role as the practice continues to expand. Position Overview The Nurse Practitioner will provide primary and acute medical care to patients in their homes and other residential settings. The provider will work independently while collaborating with our physician and administrative team. Patient schedules and geographic areas will be coordinated to make rounding as efficient as possible. Responsibilities Perform comprehensive and focused patient evaluations Manage acute and chronic medical conditions Conduct follow-up and transitional-care visits Review medications, laboratory results, imaging, and treatment plans Order appropriate laboratory testing, imaging, medications, and referrals Provide preventive and primary care services Develop and update individualized plans of care Complete timely and accurate medical documentation Communicate with patients, families, pharmacies, home health agencies, and other members of the care team Collaborate with supervising/collaborating physicians as appropriate Help identify opportunities to improve patient care and expand services Perform small in-home procedures as needed, such as IM injections, wound care, dressing changes, blood draws and other basic procedural care within scope of practice Schedule Approximately 2-4 days per month initially Flexible scheduling Opportunity for additional days as patient volume increases Potential to develop into a more substantial part-time or long-term position as ACVP expands This position may be especially attractive to an NP who already works elsewhere and would like to add a few flexible clinical days each month. Compensation Competitive compensation with flexibility in how the arrangement is structured. Depending on experience, availability, and level of involvement, compensation may be structured as: Competitive hourly rate; or Performance/profit-based compensation Final compensation structure will be discussed with the selected candidate. Qualifications Current Nurse Practitioner license in the State of Michigan Current prescribing authority as applicable Family, Adult, Adult-Gerontology, or similar primary-care background preferred Experience in primary care, internal medicine, geriatrics, hospital medicine, home-based care, or skilled nursing is preferred Comfortable evaluating and managing patients independently Strong clinical judgment and communication skills Reliable transportation and willingness to travel locally for patient visits Current driver's license and automobile insurance Ability to complete documentation accurately and promptly Preferred Candidate We are particularly interested in an NP who is: Independent and clinically confident Reliable and organized Comfortable caring for medically complex patients Interested in a flexible schedule rather than a traditional full-time position Interested in growing with a developing practice Entrepreneurial or interested in a compensation model that rewards growth and productivity About ACVP Advanced Care Visiting Physicians provides medical care for patients who benefit from receiving healthcare in their home or residential setting.
