Value-based care model seeing patients Mon-Fri, no weekends/eves Geriatric Primary Care NP/PA managing chronic conditions of patients Mon-Fri, no weekends/evenings. Providing access to patients who need it most has been the focus of HarmonyCares Primary Care Medical Group for more than 30 years. We seek a Value Based Care Nurse Practitioner to support a growing team in Kalamazoo. You will spend 30-45 min building relationships with your own patient panel addressing all chronic conditions, giving you time to achieve HEDIS and quality-focused patient outcomes. Responsibilities:
The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs. Manage chronic conditions of assigned patient panel through 30-45 min patient visits. Achieve quality outcomes meeting

measures.

Qualifications:
