This role offers a dynamic work environment across office, hospital, and dialysis settings, working closely with a collaborating nephrologist and interdisciplinary care team. The ideal candidate is clinically strong, and passionate about improving outcomes for patients with kidney disease. What You'll Do Patient Care & Clinical Responsibilities Perform patient assessments, physical exams, and medical histories Diagnose and manage acute and chronic renal conditions Order and interpret diagnostic tests Develop, implement, and adjust individualized treatment plans Prescribe medications and non-pharmacologic therapies Manage and adjust dialysis treatments as needed Perform minor procedures (e.g., suture/staple removal, line removal) Monitor patient progress and modify care plans accordingly Accurately document patient care in the medical record Collaboration & Care Coordination Work closely with nephrologists and interdisciplinary care teams Participate in rounds, care discussions, and family meetings Coordinate care across office, hospital, and dialysis settings Consult with physicians, nursing staff, and other specialists Leadership, Education & Quality Educate patients and families on treatment plans and disease management Serve as a mentor and resource for clinical staff Participate in quality assurance and performance improvement initiatives Support and engage in research and evidence-based practice Maintain current knowledge through continuing education Qualifications Graduate of an accredited Nurse Practitioner program Active Michigan RN and Nurse Practitioner licenses Current Basic Cardiac Life Support (BCLS) certification Strong clinical, communication, and interpersonal skills Ability to work independently and as part of a team Preferred Qualifications Experience with renal or dialysis patients ICU experience ANNA certification Advanced Cardiac Life Support (ACLS) certification
