Job Description

Overview We are seeking a compassionate and dedicated Primary Care Nurse Practitioner/Physician Assistant to join our healthcare team. The ideal candidate will provide high-quality, patient-centered primary care to patients across the lifespan and work collaboratively with physicians, nurses, and other healthcare professionals. This is an excellent opportunity for an experienced provider or a motivated new graduate looking to build a rewarding career in family practice. Responsibilities Provide comprehensive primary care to patients of all ages Perform physical examinations, obtain medical histories, and develop treatment plans Diagnose and manage acute and chronic medical conditions Order and interpret laboratory and diagnostic testing Prescribe medications and treatments within applicable scope of practice Provide preventive care, health screenings, and patient education Manage chronic conditions and coordinate appropriate referrals Document patient encounters accurately and efficiently in the electronic medical record Collaborate with physicians and other healthcare professionals to ensure continuity of care Promote wellness, disease prevention, and healthy lifestyle choices Qualifications Valid licensure as a Nurse Practitioner or Physician Assistant with current certification from a recognized certifying body. Demonstrated experience working with diverse populations including geriatrics, and individuals with disabilities or developmental disorders. Strong knowledge of medical terminology, physiology, diagnostic evaluation techniques, and pharmacology. Ability to work effectively as part of a multidisciplinary healthcare team Excellent client communication skills with the ability to educate patients on health coaching and treatment options while maintaining HIPAA compliance. $55.00 - $60.00 per hour
