Job Description

Nurse Practitioner for Geriatric patients in post acute settings. Are you looking for your dream job? Flexible hours? Competitive compensation? Work/life balance? If so, then this is the perfect opportunity for you. A fast growing and forward-looking multi-specialty group is seeking a full-time Nurse Practitioner with a focus on geriatrics. The position will include a focus on nursing facility visits with a mix of outpatient visits. This is a great opportunity for a provider who needs a little more flexibility with their schedule or just wants a work/life balance!
Role:
Macomb and Wayne Counties! The provider must work efficiently to provide quality primary care in multiple settings including outpatient clinic and skilled nursing/long term care facilities.
  • Focus on proactive, evidence-based treatment strategies that improve the health and well-being of individuals.
  • Communicate effectively with patients, physicians, nurses, therapists, social workers, and other staff as necessary to assure proper diagnosis and treatment.
  • Obtain, compile and record patient medical data, including health history, progress notes and results of physical examination.
  • Ensure efficient and thorough documentation of visits as required per Medicare guidelines.
  • Review and follow up with labs and diagnostic orders and determine appropriate treatment regimen.
  • You will have the support of team staff to assist with administrative and billing processes.
  • Structured physician support, training and oversight
Requirements:
Must possess valid NP license in State of Michigan. SNF/LTC experience preferred. If not, can train! The ideal candidate should show a high level of professionalism, excellent people skills, show empathy and compassion, deliver high quality care, and be able to work well in a team setting.
