Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and other vulnerable populations in their homes. You will complete detailed health risk assessments, review medications, address care gaps, and educate patients on chronic disease management and prevention. Using Signify Health's technology and data tools, you'll document encounters, support value-based care, and collaborate with remote clinical teams.
