Must have a valid Minnesota State driver's license, current insurance or be insurable. Must be current via Minnesota State License of Nurse Practitioner (NP) or Physician Assistant (PA-C). National Board Certification.

Summary:

Provide primary health care services within the tribally operated, community-based clinics.

Education Requirements and Experience:

Minnesota State License of Nurse Practitioner (NP) or Physician Assistant (PA-C) is required. Two (2) years of primary health care experience in a tribally operated, Indian Health Service (IHS) program, or a rural clinic is preferred.

Additional Qualifications and Skills:

Knowledge of the Electronic Health Record (EHR) system is required. Ability to travel to all tribal clinic settings is required. Ability to travel out of state when required. Good attendance and punctuality a must. Excellent office management and computer skills. Excellent communication and customer service skills (both verbal and written). Understanding and sensitivity to Native American culture. Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe safety sensitive positions.

Duties/Responsibilities:

Provide high-quality health care services within scope of practice designated by the specific certification held by the provider. Accessible and available health services. Education of and effective communication with those served, concerning the diagnosis and treatment of their medical conditions, appropriate preventive measures, and use of the health care system. Treatment that is consistent with clinical impressions and working diagnoses. Continuity of care. Appropriate, accurate and complete medical record entries. Adequate and appropriate transfer of information when clients are referred to another health care provider. Assess real or potential health hazards and current health status, including analysis of health behavior related to lifestyle and culture. Devise, implement and evaluate plans of care utilizing sound clinical judgments based on assessment of the physical, psycho-clinical judgments based on assessment of the physical, psychological, emotional, societal and environmental needs of the patients. Assist individuals to assume responsibility for the prevention of illness and the promotion, maintenance and restoration of health. Consult, refer and collaborate with other disciplines involved in the delivery of total client care Utilize the resources of the Reservation community. Promote collegial relationships with other clinical staff and providers in the community. Demonstrate evidence of continuing professional growth by involvement in peer review and the review of protocols and procedures; participate in continuing education. Demonstrate awareness of issues relating to nursing practice and health care. Participate in community education and other professional activities. Meet routinely with collaborating physician as per IHS protocol. Utilization of the Electronic Health Record system, including: coding, referral tabs and templates. Apply for Minnesota prescribing privileges. Attend and/or conduct all mandatory meetings and trainings as required. Maintain confidentiality and professional ethics at all times. Adhere to policies governing staff conduct. All other related duties as assigned.
