Job Description
We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, LTC, and ALFs. Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans. Order diagnostics, prescribe medications, and educate patients, families, and staff. Collaborate with care teams to ensure seamless, patient-centered care. Maintain accurate documentation and support quality initiatives.
Qualifications:
Current MN NP license, board certification, and 1+ year experience preferred. Strong geriatric and primary care skills required.Benefits:
