Job Description

PRIMARY RESPONSIBILITIES
Evaluate and treat center patients in accordance with standards of care Care coordination with other providers, specialists, and care facilities Perform comprehensive in-home assessments for identified patients of the practice and assisting the primary care team in better managing chronic diseases Complete physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings Communicate assessment findings that will be used to inform the PCP of potential gaps in care Will have support of Medical Assistants while seeing patients to allow more time for direct patient care Highly relationship oriented and ability to passionately serve the aging and elderly population Educate patients on topics such as disease process, medication, and compliance Comply with all HIPAA regulations and maintain security of protected health information
QUALIFICATIONS
Must have or be eligible to have a current and unrestricted NP or PA license Experience as an Independent Practitioner in Out-patient setting Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs) Embrace teamwork and the opportunity to collaborate with talented colleagues Certificate of completion from an accredited Nurse Practitioner Program or Physician Assistant Program Current DEA certificate required prior to start date BLS certification At least two-year experience practicing as an N.P. or P.A. Two years of experience in Primary Care or Adult/Geriatrics
