Monday-Friday, 8:00 a.m•5:00 p.m. UNC Primary Care at Graham is seeking experienced Nurse Practitioner or Physician Assistant to join our primary care team at this brand new, state-of-the-art facility. Ideal Candidate Qualifications ?Board Certified or Board Eligible in Family Medicine or Internal Medicine ?Active, unrestricted North Carolina medical license (or eligibility to obtain one) ?Strong interest in caring for medically complex adult patients ?Demonstrated experience and interest in addressing mental health needs within a primary care setting Clinical Services Provided ?Comprehensive preventive healthcare ?Acute and sick care ?Annual physicals and wellness visits ?Immunizations ?Women?s health services ?Management of acute and chronic conditions, including diabetes, hypertension, and asthma ?Routine screening and diagnostic testing Qualifications ?Family Medicine Advanced Practice Provider (NP or PA) ?Active North Carolina license (or eligibility to obtain one) ?Minimum of 2 years of clinical experience About the

Education Requirements:

? Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.

Licensure/Certification Requirements:

? Licensed as an Advanced Practice Provider (i.e., Nurse Practitioner, Physician Assistant, Clinical Pharmacist Practitioner, Certified Nurse Midwife, Certified Registered Nurse Anesthetist) in the state of North Carolina. ? Board certification if required by the Department. DEA.
