Nurse Practitioner - PRN Revella Health Greensboro, NC Job Details Contract 8 hours ago Benefits Flexible schedule Qualifications Community health & wellness Collaborate with healthcare professionals Nurse Practitioner (NP) Advanced practice provider Patient care leadership Full Job Description Nurse Practitioner (NP) - Community Mini Clinics Revella Health is expanding its community-based preventive screening clinics across the North Carolina Triad, including Greensboro, Winston-Salem, and High Point. We are looking for experienced and dependable Nurse Practitioners to provide clinical leadership during scheduled mini clinic events. These clinics are designed to bring convenient preventive screenings and health education directly into the communities we serve. Position Summary As the senior clinical provider on-site, the Nurse Practitioner oversees clinical activities during Revella Health mini clinics and supports the clinical team throughout the event. This role includes completing designated screenings and assessments, educating members according to established protocols, providing referral recommendations, and responding to findings that require clinical escalation. Key Responsibilities Complete PHQ-9 assessments for members with positive PHQ-2 screening results. Provide smoking cessation counseling when indicated. Conduct A1c testing, blood pressure checks, and depression screenings when needed. Educate members regarding screening results and reinforce appropriate PCP follow-up. Provide referral recommendations consistent with established Revella Health protocols. Serve as the senior clinical resource for RNs, CMAs, and other clinic staff during clinic operations. Evaluate and manage findings escalated as urgent or emergent according to established protocols. Conduct member follow-up calls when required. Maintain timely and accurate documentation of screenings, education, escalations, referrals, and follow-up. Collaborate with the clinic team to maintain an organized and efficient flow throughout the clinic day. Qualifications Active, unencumbered Nurse Practitioner license in North Carolina required. Strong clinical assessment and decision-making skills. Effective communication and documentation skills. Comfortable providing clinical oversight in a community-based preventive health setting. Professional, dependable, and able to work independently within established clinical protocols. Reliable transportation and ability to travel to scheduled clinic locations throughout the Triad. Ability to follow Revella Health screening, documentation, and escalation procedures. Role Scope This position is focused specifically on preventive health screening services, member education, clinical oversight, escalation management, and referral recommendations. Revella Health Nurse Practitioners do not diagnose or treat medical conditions, perform medication reconciliation, or prescribe medications as part of this role.
