One Health currently operates 35 primary care offices across the greater Charlotte and Winston-Salem areas of North Carolina. Our specialties include Family Medicine, Internal Medicine, Pediatrics, Sports Medicine, Rheumatology, OB/GYN, and Urgent Care. Position Highlights We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our expanding Transitional Care Management Team. This role focuses on caring for high-risk patients recently discharged from the hospital, with the goal of improving outcomes, reducing readmissions, and ensuring a smooth transition back to the community. Responsibilities Provide Transitional Care Management services to recently discharged patients. Collaborate closely with hospitalists, Emergency Department providers, and Case Management teams at Atrium Health Lake Norman Hospital. Work as part of a multidisciplinary team including Registered Nurses, LPNs, Pharmacists, and Social Workers. Office hours are Monday through Friday. Passionate about providing patient-centered care across a broad spectrum of disease states and chronic conditions. Experienced in hospital-based or Emergency Department settings (preferred).
