About the Role You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities.
Your responsibilities include:
Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day Collaborating with interdisciplinary teams to optimize patient outcomes Ensuring smooth transitions of care with hospice, palliative care, and home care agencies Maintaining accurate and timely documentation in our EMR system (Gehrimed) Why Choose TeamHealth?

Expected compensation is estimated range of $93,000 to $149,000 annually with no cap on productivity income potential. This full-time role is benefit eligible.
