Job Description

We are seeking to hire Nurse Practitioner to work in Skilled Care. In this role, you won't just be treating symptoms; you'll be providing personalized, comprehensive primary care to seniors and individuals with complex medical needs within the comfort of their own residences. Our NPs provide primary care services to seniors and patients with complex health needs. You will conduct clinical assessments and develop personalized care plans in skilled nursing . You will collaborate with interdisciplinary teams to ensure seamless care transitions and improved patient outcomes as well as manage chronic conditions and acute visits with a focus on preventative health. Performs evidence-based medical care by planning, directing, and evaluating patient medical needs Examines and treats chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses Orders and interprets laboratory and diagnostic testing as needed Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care Provides referrals to other healthcare practitioners when necessary Ensures patient medical records are kept up to date
Qualifications:
Active state Nurse Practitioners license Required to have worked in Skilled Care as RN or NP or worked in Medical/Surgical Unit working with Geriatric Patients. Will consider New NP if they have at least 3 years of RN experience in Skilled Care or Medical Surgical Units. Current Nurse Practitioner National Board Certification Active state DEA licensure Strong knowledge base of HIPAA and OSHA regulations This position is days Monday - Friday. No Nights or weekends.
