Job Description

Provide high-quality, patient-centered care to older adults in skilled nursing and post-acute facilities in Ossipee, NH, as part of a collaborative, team-based practice.
Locations Primary:
Ossipee, New Hampshire Additional opportunities nationwide:
IL, MA, MI, NC, OH, WI
Schedule & Clinical Duties Full-time positions with flexible scheduling Typical shifts: six-hour facility rounds beginning between 7:00-10:00 a.m. No on-call or overnight responsibilities Perform daily rounds, manage acute and chronic conditions, prescribe medications and treatment plans, and coordinate care with facility staff and families Efficient documentation using streamlined EHR with AI scribe, voice dictation, and company-provided iPad with LTE Access to UpToDate for clinical guidance Compensation & Benefits Competitive base pay with uncapped, productivity-based earning potential Up to $50,000 in long-term retention incentives Performance bonuses and sign-on bonus (eligible candidates) Paid Time Off 401(k) with employer match Medical, dental, vision, life, and disability insurance (eligible providers) CME allowance and licensure reimbursement Company-provided iPad with LTE and lab coat Relocation assistance available for eligible candidates Qualifications Active, unrestricted Nurse Practitioner license National certification (ANCC, AANPCB, PNCB, AACN, or equivalent) Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred Apply if you are a clinically driven NP committed to improving the health and quality of life of older adults in facility-based settings.
