The provider will deliver comprehensive, patient-centered therapeutic healthcare services within the scope of practice outlined by state law, licensing regulations, and practice agreements. This role focuses on diagnosing and managing acute and chronic health problems, promoting health maintenance across the continuum of care, and coordinating seamlessly with other healthcare professionals in a strictly traditional primary care setting. Duties and Responsibilities Clinical Care & Diagnostics

• Function independently and as a member of an interdisciplinary team to plan, deliver, and evaluate healthcare for a defined patient population.
• Conduct comprehensive reviews of medical documents, lab results, hospital discharge summaries, and consult notes to ensure continuity of care.
• Analyze and interpret patient histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses.
• Diagnose and treat acute episodic healthcare problems (illnesses, infections, injuries) as well as manage chronic conditions.
• Order, perform, and interpret the results of diagnostic tests, including complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays).
• Perform pre-operative evaluations, including surgical clearance, and provide post-operative follow-up care for outpatients.
• Document patient assessments and plans of care accurately in the medical record and generate appropriate health reports for outgoing correspondence. Treatment & Prescribing
• Develop evidence-based treatment plans utilizing scientific rationale, standards of care, and professional practice guidelines.
• Perform medical diagnostic and therapeutic procedures; prescribe, dispense, or deliver medications as legally authorized and outlined in collaborative practice agreements.
• Counsel patients about drug regimens, potential side effects, and interactions with food supplements, OTC medications, and herbal remedies.
• Detect and respond to adverse drug reactions, paying special attention to vulnerable populations (e.g., infants, pregnant/lactating women, and older adults). Patient Education & Coordination
• Educate patients and families regarding medications, treatments, and self-management of acute or chronic illnesses.
• Determine clinical care requirements for the patient population, including telephone triage and ongoing follow-up.
• Promote health maintenance and recommend interventions to modify behaviors associated with health risks.
• Provide oral and written communication with referring physicians, pharmacists, and home healthcare agencies, and coordinate follow-up visits and procedures as required.
• Recommend community resources to meet patient and family needs. Operational Support
• Collaborate with clinical staff and administration to monitor and evaluate the quality of patient care.
• Supervise clinical activities of Salem site staff and manage patient workflows in the absence of the Clinical Director or Clinical Manager.
• Participate in an on-call rotation to ensure patient needs are met after hours.
• Maintain skills and stay current with advancements in medical equipment, treatment materials, and emerging healthcare trends. Qualifications

Education:

Master's degree or equivalent from an accredited APRN program, OR graduation from an accredited educational training program for Physician Assistants.

Certification:

Current certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA). Specialty certification in Family Medicine or Adult/Internal Medicine is preferred.

Licensure:

Active, current, and unencumbered license to practice as an APRN or PA in the state of New Hampshire.

Prescriptive Authority:

Current DEA certification and eligibility.

Life Support:

Current BCLS (Basic Cardiac Life Support) / CPR / AED certification.

Experience:

3-5 years of clinical experience in a traditional primary care, public health, or outpatient clinic setting is strongly preferred.

Skills:

• Ability to maintain confidentiality and handle sensitive medical information with absolute discretion.
