Supervising Physician Position Summary Total Patient Care, LLC is seeking a compassionate and experienced Nurse Practitioner (APN) to join our Internal Medicine practice. The ideal candidate will provide high-quality primary care to adult patients while working collaboratively with our physicians and clinical team to promote exceptional patient outcomes. Responsibilities Evaluate, diagnose, and treat acute and chronic medical conditions. Perform annual wellness visits, routine physicals, and follow-up appointments. Order and interpret laboratory and diagnostic tests. Prescribe medications and develop individualized treatment plans. Educate patients on disease prevention, medications, and healthy lifestyle choices. Managing chronic conditions. Coordinate care with specialists and other healthcare providers. Maintain accurate documentation in the electronic medical record (EMR). Participate in quality initiatives and value-based care programs. Qualifications Current New Jersey APN license. National Board Certification (AANP or ANCC). Current DEA and CDS registration. BLS Certification. Minimum of 1-2 years of experience in Internal Medicine, Primary Care, or Family Medicine preferred. New graduates will gain experience and mentorship from our established professionals on staff. Strong clinical judgment, communication, and patient education skills.
