Responsibilities Provide comprehensive primary and preventive care to patients across the lifespan, including assessment, diagnosis, and management of acute and chronic conditions. Perform complete histories and physical examinations, order and interpret laboratory and diagnostic tests, and develop evidence-based treatment plans. Prescribe medications, administer or oversee immunizations, and conduct routine health screenings in accordance with clinical guidelines and state regulations. Deliver patient and family education on diagnoses, medications, treatment plans, and lifestyle modifications to support disease prevention and health promotion. Utilize electronic health record systems to document encounters, update problem lists and medication lists, and ensure accurate, timely, and compliant documentation. Collaborate with pharmacists, physicians, nurses, behavioral health providers, and other members of the care team to coordinate patient care and ensure continuity across settings. Participate in quality improvement, population health initiatives, and adherence to clinical pathways aimed at improving outcomes and reducing avoidable healthcare costs. Provide care through both in-person clinic visits and telehealth/virtual visits as appropriate, ensuring safe, effective, and patient-centered remote care. Monitor and manage chronic diseases such as diabetes, hypertension, hyperlipidemia, asthma, and COPD, adjusting treatment plans based on clinical response and guidelines. Maintain current licensure, certifications, and clinical competencies, and comply with all relevant legal, regulatory, and organizational policies and procedures.
