About Us We are a compassionate, patient-centered medical practice based in East Hanover, New Jersey, specializing in mobile healthcare services. Our dedicated team delivers high-quality palliative care directly to patients in private homes and assisted living facilities throughout the region. We believe in dignity, comfort, and continuity of care—wherever our patients call home. Role Overview We're looking for a compassionate and experienced Nurse Practitioner (NP) with a background in Palliative Care to join our mobile medical team. This position involves traveling to Private Homes and Assisted Living Facilities to support patients with serious illnesses by managing chronic and acute conditions, prioritizing comfort, quality of life, and symptom relief. The role offers independent practice with flexible scheduling, backed by a supportive and collaborative clinical team, while building meaningful relationships with patients and their families. Responsibilities
- Provide comprehensive palliative care to patients with chronic or life-limiting illnesses
- Conduct assessments, develop care plans, and manage symptoms
- Collaborate with physicians, caregivers, and facility staff to ensure continuity of care
- Educate patients and families on disease progression, treatment options, and end-of-life planning
- Document visits and maintain accurate medical records in compliance with regulatory standards
- Participate in interdisciplinary team meetings and contribute to quality improvement initiatives
Qualifications:
- Active Nurse Practitioner license in the state of New Jersey
- Certification in palliative care, oncology or relevant experience strongly preferred
- Minimum of 1 year clinical experience in geriatrics, hospice, or chronic disease management
- Strong communication and interpersonal skills
- Ability to work independently and travel to patient locations
- Reliable transportation and valid driver's license What We Offer
