Position Overview We're looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient primary care team. This role is responsible for the overall care and management of patients, including new and existing patient visits, managing acute and chronic conditions, providing preventive care, ordering and reviewing labs and diagnostic tests, prescribing medications, and coordinating follow-up care. You'll work closely with physicians and clinical/administrative staff to deliver efficient, high-quality patient care. Key Responsibilities Evaluate, diagnose, and treat primary care patients See new and follow-up patients Review patient medical history and perform physical examinations Diagnose and manage common acute and chronic conditions Manage chronic conditions such as hypertension, diabetes, and hyperlipidemia Order labs and diagnostic tests and review results as needed Prescribe medications and develop treatment plans based on patient condition Provide annual physicals and preventive care Manage health screenings Review lab and diagnostic results and follow up with patients Refer patients to specialists as needed Educate patients on their condition, treatment plan, medications, and lifestyle Document patient visits accurately and promptly in the EMR/EHR Collaborate with physicians, medical assistants, and front desk staff to maintain smooth patient flow Comply with relevant medical regulations, patient privacy, and patient safety standards Qualifications Required Active, unrestricted New Jersey license as a Nurse Practitioner (NP) or Physician Assistant (PA) NP Board Certification or PA Board Certification (NCCPA), as applicable
For PAs:
an active Collaborative/Supervising Physician Agreement, or ability to establish one DEA Registration, or ability to obtain one if required for the role Ability to assess patients and make sound clinical decisions Strong patient communication and interpersonal skills Ability to work efficiently in a fast-paced outpatient clinic setting Comfortable using EMR/EHR systems Preferred Experience in primary care or internal medicine Experience in family medicine or urgent care Experience managing patients with acute and chronic conditions Experience with preventive care and patient education Bilingual in English and Korean preferred Schedule & Location DoctorHere Primary Care Clinic, 886-926 Prospect Street, Glen Rock, NJ 07452
