Schedule- Part-time 2 days per week, 12 hours a week Must have an NP license and a DEA License 1099 position Redwood Health Partners is seeking an experienced and mission-driven Nurse Practitioner to join our integrated care team serving residents in a Senior Living community. You will work in a fully integrated senior care model through Connect4Life , an innovative partnership between Redwood Health Partners and Juniper Communities. What You'll Do Conduct new patient consultations and comprehensive health assessments Perform physical exams and manage acute and chronic conditions Develop individualized treatment plans for medically complex geriatric patients Coordinate care transitions across hospital, rehab, and community settings Collaborate closely with physicians, therapists, nursing staff, and specialists Order, interpret, and manage diagnostic testing Provide patient and family education on ongoing care plans Maintain accurate, compliant documentation in a state-of-the-art EHR Participate in quality improvement initiatives focused on reducing avoidable hospitalizations What Makes You a Great Fit Master's or Doctorate degree as a Nurse Practitioner Active Pennsylvania NP license DEA License Minimum of 5 years of clinical experience (geriatric experience strongly preferred) Strong clinical judgment in managing multiple co-morbidities Skilled in care coordination and interdisciplinary collaboration Proficient in EHR documentation Deep understanding of HIPAA and regulatory standards A genuine passion for improving quality of life for older adults Schedule & Structure 1099 Independent Contractor position Flexible schedule: 3 days per week Opportunity for additional hours based on availability and community needs This role is ideal for a seasoned NP seeking meaningful part-time work, supplemental income, or the opportunity to practice high-quality geriatric medicine in a coordinated care environment.
