How YOU will benefit Quality 1:1 care to make a lasting impact on patients and families Provide care that is centered on comfort, dignity, and goals of care Greater work/life balance with flexible scheduling options Less time on your feet compared to other settings Ability to work independently while also having interdisciplinary team support Job stability and regular advancement opportunities with a growing company As a Nurse Practitioner You will: Provide compassionate medical care and symptom management Perform assessments, order/ interpret diagnostic tests, and develop care plans Prescribe medications, manage treatments, and coordinate hospice services Conduct and document face-to-face visits for eligibility and recertification Collaborate with physicians and IDT team to align care with patient goals. Participate in care planning and coordinate with team and external providers Maintain accurate clinical documentation and ensure compliance regulation Educate patients & families on medical, emotional, and spiritual needs Contribute to quality improvement efforts and professional development Serve as a clinical resource and liaison to community partners Responsibilities Facilitate new patient consultation and health history review Perform physical examinations and screenings of patients Order and administer tests including bloodwork Review and interpret test results Diagnose health problems Detect changes in patient condition Administer medications Operate medical equipment Recommend further treatment options or future appointments if needed Provide referrals to other practitioners when necessary Provide after-care/ongoing care information and resources to patients Ensure patient records are kept up to date including test results, immunizations, condition changes, etc. Deliver quality, physician-level care while complying with company policies, safety regulations, and productivity metrics All other duties as assignedQualifications Education:
Master's degree in nursing Current state nurse practitioner license required 5+ years nursing experience requiredQualifications:
Strong knowledge base of HIPAA and OSHA required Strong interpersonal communication skills, both verbal and written Patient, compassionate nature Ability to operate as part of a team, alongside physicians and other medical staff Proficiency in medical instruments and equipment Proficiency in computer-based data management programs and information systems preferred Prescriptive authorization requiredWork Requirements:
