We bring psychiatric care, psychotherapy, clinical coordination, and advanced treatment pathways directly into the long-term care communities we serve. As we expand our presence in New York, we are looking for clinicians who want to play a meaningful role in both patient care and the growth of a new model of care. The Role Siyata Health is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care to older adults across assisted living and long-term care communities in the Albany / Capital Region . This is a highly autonomous clinical role for a provider who is comfortable managing a diverse psychiatric population while collaborating closely with facility teams, families, therapists, and the broader Siyata clinical team. The PMHNP will establish ongoing relationships with residents rather than simply providing episodic psychiatric consultations, with an emphasis on continuity, thoughtful clinical care, and improving each patient's overall behavioral health. Responsibilities Conduct comprehensive psychiatric evaluations and ongoing follow-up visits Diagnose and manage psychiatric and behavioral health conditions Develop individualized treatment plans and provide psychopharmacologic management Monitor medication effectiveness, side effects, and changes in clinical presentation Collaborate with therapists, physicians, nursing teams, facility staff, residents, and families Review relevant medical histories, medications, laboratory results, and clinical documentation Provide timely clinical documentation and maintain appropriate standards of care Help identify residents who may benefit from additional behavioral health services or advanced treatment options Participate in care coordination and interdisciplinary discussions as appropriate Build strong, professional relationships with the communities and clinical teams we serve Ability to travel locally between assigned communities within the Albany / Capital Region Required Qualifications Licensed Nurse Practitioner in the State of New York Psychiatric-Mental Health Nurse Practitioner certification Graduate degree from an accredited Nurse Practitioner program Current prescribing authority and applicable controlled-substance credentials Strong clinical judgment and ability to practice with a high degree of independence Excellent communication, documentation, and interpersonal skills Strongly Preferred Experience treating older adults Experience in geriatric psychiatry or behavioral health Experience working within assisted living, skilled nursing, memory care, or other long-term care settings Familiarity managing medically and psychiatrically complex patients Experience collaborating with interdisciplinary care teams Compensation & Benefits Competitive compensation of $85-$100 per hour , based on experience and qualifications Health, dental, and vision insurance Paid time off Paid holidays Malpractice insurance coverage Continuing education support Professional licensing and credentialing support Mileage reimbursement for required travel between assigned communities Flexible and supportive clinical environment Opportunity for increased responsibility and professional growth as Siyata Health expands across the region Why Siyata Health This is an opportunity to do more than step into an established clinical position.
