Nurse Practitioner - Nutrition Practice Initial Consultation & Clinical Coordination Job Overview We are seeking a detail-oriented and collaborative Nurse Practitioner to join Well-Choices as a clinical gateway for our telehealth nutrition practice. In this role, you will conduct initial patient consultations, order and interpret diagnostic labs, establish Remote Therapeutic Monitoring (RTM) protocols, and facilitate seamless transitions to our registered dietitian team for long-term nutritional care. You will be instrumental in ensuring clinical clarity, safety, and an exceptional patient onboarding experience while supporting our dietitians with evidence-based clinical decision-making.
Conduct comprehensive virtual assessments during first-visit appointments, reviewing medical history, current medications, relevant lab values, and health goals to establish clinical baseline and identify lab needs.
Diagnostic Lab Ordering & Interpretation :
Order appropriate labs (metabolic panels, lipids, glucose, micronutrient levels, inflammatory markers) based on patient presentation; review and interpret results to guide dietary intervention strategy.
RTM Setup & Patient Education :
Educate patients on Remote Therapeutic Monitoring protocols (food logging, weight tracking, symptom monitoring, biometric data submission); ensure patient proficiency with tracking platforms and clarify monitoring expectations before dietitian handoff. Handoff to
Dietitian Team :
Prepare clear clinical summaries for assigned registered dietitian, highlighting key findings, lab results, medical flags, and RTM parameters to ensure continuity of care.
Telehealth Coordination :
Optimize patient experience with telehealth platform navigation, troubleshoot technical barriers, and ensure patients are comfortable with asynchronous data submission and secure messaging.
Clinical Documentation & Compliance :
Maintain accurate, timely documentation in EMR/EHR systems; ensure HIPAA compliance and proper coding for initial consultation visits.
Collaborative Care :
Work closely with our dietitian team and external healthcare providers (PCPs, specialists) to address clinical questions, coordinate care adjustments, and communicate patient progress. Required Skills & Experience Current NY licensure Proficiency with EMR/EHR systems; experience with HubSpot, Google and similar platforms is a plus. Strong knowledge of clinical nutrition, metabolic health, and common nutrition-related conditions (diabetes, hypertension, cardiovascular disease, weight management, autoimmune conditions). Competency in telehealth patient assessment techniques Experience ordering and interpreting common diagnostic labs related to nutrition and metabolic health. Familiarity with Remote Therapeutic Monitoring (RTM) concepts and asynchronous patient data review. Excellent written and verbal communication; ability to clearly document clinical findings and transition care to team members. Patient-centered approach with strong interpersonal skills and ability to educate patients on technology and monitoring expectations..
