About Optum Care at Home The Optum Care at Home (OAH) program provides longitudinal, integrated care to high-risk patients where they live. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll partner with patients, caregivers, and the broader care team to manage complex medical conditions, support recovery, and help patients maintain their independence and quality of life. This role offers a unique opportunity to practice at the top of your license while building meaningful relationships with patients and making a lasting impact in the communities you serve. Primary Responsibilities Conduct comprehensive in-home assessments, including:
Medical history review
Medication reconciliation
Physical examinations and vital signs
Evidence-based screenings and point-of-care testing, as appropriate
Manage patients experiencing acute illnesses or exacerbations of chronic conditions, including diagnostics, testing, and interventions
Support patients during short-term rehabilitation and recovery episodes
Maintain accurate and complete clinical documentation, including ICD-10 coding
Collaborate closely with physicians, nurses, care coordinators, and other interdisciplinary team members
Facilitate advanced care planning and goals-of-care discussions
Educate patients and caregivers on disease management, treatment plans, and healthy lifestyle practices
