Overview We are seeking an experienced Nurse Practitioner and Physician Assistant to join our a new Home Health Agency in Brooklyn, NY. This is an in-person position working as a 1099 where you'll provide care through a telehealth platform in a modern, collaborative clinical environment.

Responsibilities Conduct patient evaluations and follow-up visits through a telemedicine platform Diagnose, treat, and manage a variety of acute and chronic conditions Develop and implement individualized treatment plans Maintain accurate and timely electronic medical records Collaborate with physicians and clinical staff to ensure exceptional patient care Qualifications Active Nurse Practitioner or Physician Assistant license in New York Must be credentialed with PECOS Strong clinical assessment and communication skills Telehealth experience is a plus Home Health experience is a plus Ability to thrive in a fast-paced, patient-focused environment Ability to speak Spanish is a plus Compensation & Benefits $130,000-$180,000 annual salary
