Family Choice of New York is seeking a compassionate, experienced, and clinically astute Nurse Practitioner (NP) or Physician Assistant (PA) to join our Care Management team. This position plays a critical role in providing comprehensive care management services for members with complex chronic conditions, ensuring high-quality outcomes through collaboration, assessment, education, and care coordination.

Qualifications:

Current New York State licensure as a Nurse Practitioner (NP), or Physician Assistant (PA), Board Certification required, and CPR Certification required

Preferred Experience:

Geriatric nursing and/or home care experience, Chronic disease management, Care of medically complex patients with multiple chronic conditions The Care Management APP serves as a key clinical leader within the interdisciplinary care team, providing in-home and facility-based services to members with complex healthcare needs. This role combines advanced clinical assessment, chronic disease management, care coordination, patient education, and collaborative care planning to improve member outcomes, enhance quality of life, and support members in remaining safely within their own environment whenever possible. Pay:

$125,000.00 - $142,000.00 per year
