What We Offer 3-day workweek $120,000 annual salary Primary location in Arverne/Far Rockaway, Queens Meaningful work serving an underserved and diverse patient population Collaborative, multidisciplinary healthcare environment Opportunity to make a direct impact in HIV prevention, treatment, education, and long-term patient care What You'll Do As our Infectious Disease/HIV Nurse Practitioner, you will provide comprehensive, patient-centered care with a primary focus on individuals living with HIV/AIDS and patients requiring infectious disease services.
Responsibilities include:
Evaluate, diagnose, treat, and provide ongoing management for patients with HIV/AIDS, Hepatitis C, and other infectious diseases Provide comprehensive primary and preventive healthcare Develop individualized treatment plans based on clinical findings and diagnostic results Prescribe medications within scope of practice Provide physical examinations, immunizations, screenings, and preventive services Educate and counsel patients regarding medication adherence, disease management, risk reduction, lifestyle changes, and overall wellness Coordinate specialty consultations, diagnostic services, referrals, and follow-up care Collaborate with physicians, nurses, social workers, counselors, pharmacists, behavioral health professionals, nutritionists, and other members of the healthcare team Participate in quality improvement, patient education, and community health initiatives Maintain accurate and timely clinical documentation Provide coverage at other Joseph P. Addabbo Family Health Center locations when needed based on operational and patient-care needs What We're Looking For Graduate of an approved Nurse Practitioner program Current New York State Nurse Practitioner license Knowledge of PrEP and PEP Current BLS certification Experience or strong clinical knowledge in HIV/AIDS, Hepatitis C, and infectious disease care Strong patient education and care-coordination skills Ability to work effectively within a multidisciplinary healthcare team Commitment to compassionate, culturally responsive, patient-centered care A genuine interest in improving healthcare access and outcomes for underserved communities Why Addabbo?
