Job Description
Oak Street Health seeks a Nurse Practitioner to provide compassionate primary care to adult Medicare patients in underserved communities. You will conduct comprehensive assessments, manage chronic and acute conditions, and develop personalized care plans with a focus on prevention. Collaborate closely with physicians, nurses, and care teams to coordinate services, educate patients and families, and improve outcomes.
