What You'll Do Provide comprehensive adult primary care: annuals, follow-ups, sick visits, preventive screenings Diagnose and manage chronic conditions (HTN, DM, asthma/COPD, thyroid disorders, hyperlipidemia, etc.) Coordinate care with specialists and oversee referrals when needed Educate patients clearly and partner with them on achievable care plans Maintain accurate, timely documentation and collaborate with the clinical team What We're Looking For MD/DO from an accredited medical school or NP with experience.
