Job Description

Seniority Healthcare is seeking a part-time Nurse Practitioner or Physician Assistant to join our team in New City, NY for 16 hours a week. In this role, you will provide comprehensive care to a high-need, high-risk geriatric population. The ideal candidate has a background in Internal Medicine, Family Medicine, or Geriatrics, and can work independently while collaborating with a multidisciplinary team. This position offers a balance of direct patient care and remote responsibilities-such as prescription refills, lab reviews, and patient communication-allowing for a flexible and rewarding schedule. Key Responsibilities Conduct thorough physical exams and wellness assessments for seniors Follow up on care plans to ensure the best possible outcomes Propose and manage treatment plans for chronic and acute conditions Maintain accurate patient records and manage appointments effectively Educate patients on preventative care, treatments, and healthy lifestyle choices Collaborate with a supportive team dedicated to holistic care Qualifications Completion of an accredited NP or PA program At least one year of experience in geriatrics, internal medicine, or family practice Strong communication, active listening, and analytical skills Compassionate and committed to providing patient-centered care Motivated to serve a geriatric population with complex care needs Ability to work 16 hours a week, 2 days onsite at an assisted living facility.
