Key Responsibilities
Complete comprehensive patient assessments, including:
Review of medical history and medications
Physical examinations and vital signs
Appropriate screenings and point-of-care testing
Support members through short-term sub-acute rehabilitation stays
Maintain thorough and accurate clinical documentation, including ICD-10 coding
Partner closely with interdisciplinary care teams to coordinate treatment plans
Lead discussions around advance care planning and serious illness care
