Required Qualifications For Nurse Practitioners:

Current ANCC or AANP certification in

Family, Adult, Geriatric, or Adult-Gerontology Primary Care For Physician Assistants:

Active NCCPA certification and applicable state licensure Additional requirements: Unrestricted state license (or ability to obtain before start date) Valid driver's license with reliable transportation Active DEA license/prescriptive authority, or ability to obtain after hire in accordance with state requirements Ability to establish a collaborative practice agreement when required Ability to work in a variety of clinical settings and perform essential job functions Preferred Qualifications One or more years of APC experience, preferably in long-term care Experience in geriatric care, senior living, home care, or long-term care environments Familiarity caring for patients with complex medical, behavioral, social, or functional needs Knowledge of chronic disease management and geriatric medicine Experience facilitating advanced illness and end-of-life conversations Comfort using electronic medical records and healthcare technology

Compensation:

Hourly compensation for this position typically ranges from $53 to $79 per hour, based on factors such as location, experience, education, and other job-related considerations.
