As a School Based Health Nurse Practitioner/Physician Assistant, you'll deliver primary care, health promotion, and illness prevention in a school-based setting and other clinical sites. Responsibilities include conducting exams, managing acute and chronic conditions, administering vaccines and medications, and collaborating with a multidisciplinary team. You will document encounters, adhere to clinical guidelines, participate in quality improvement, and support patient-centered care models. Qualified candidates must have NY licensure, board certification, and relevant clinical experience, with preferred experience in community health and EMR systems.
