Responsibilities:

The Nurse Practitioner - INTERNAL

MEDICINE

- will work with patients ranging from 18 to advanced age Order and analyze diagnostic tests Counsel patients about options for their medical conditions Provide non-life-threatening emergent care to patients of all ages, and management of acute and chronic illnesses. Conduct comprehensive health assessments, including physical examinations, medical history reviews, and ordering and interpreting diagnostic tests. Develop and implement treatment plans based on evidence-based guidelines and patient-specific needs. Manage overall patient care Educate patients on preventative care and disease management Accurate and clear updates and records changes in health status in EMR Make appropriate referrals to other health professionals and community agencies among others Must have a high degree of professionalism in all communications Excellent communication skills with ability to work individually and as part of a team Understanding that the patient is our highest priority
